1. About these terms

These Terms & Conditions ("Terms") govern the supply of supports and services by Guided Feet ("we", "us", "our") to you ("the participant", "the client", "you"). By engaging Guided Feet verbally, in writing, by signing a Service Agreement, or by accepting services from us you agree to be bound by these Terms together with any separate Service Agreement we put in place with you.

2. Our services

Guided Feet provides person-centred disability, aged care and private support services across Perth and surrounding suburbs, including (without limitation) social supports, personal care, transport, community participation, domestic assistance, meal preparation and complementary wellbeing services. The exact supports we deliver to you are recorded in your individual Service Agreement and schedule of supports.

3. NDIS participants

Where you are an NDIS participant, our services are delivered in line with the NDIS Act 2013, the NDIS Practice Standards, the NDIS Code of Conduct and the current NDIS Pricing Arrangements & Price Limits. Our charges will not exceed the relevant NDIS price limits unless agreed in writing with you.

4. Fees and payment

  • Our fees are set out in your Service Agreement or quote.
  • NDIS-funded supports will be claimed against your plan in accordance with your nominated management type (self-managed, plan-managed or NDIA-managed).
  • Private and aged care clients will be invoiced after each shift or monthly, payable within 7 days of the invoice date.
  • Travel, public holiday and short-notice loadings may apply in line with the NDIS Pricing Arrangements.

5. Cancellations and no-shows

We ask for at least 7 clear days' notice to cancel or reschedule a booked shift. Where less notice is given, or you are not present and contactable at the agreed time, a short-notice cancellation fee of up to 100% of the shift may be charged, consistent with the NDIS Pricing Arrangements.

6. Your responsibilities

  • Treat our workers with courtesy and respect.
  • Provide a safe working environment, free of hazards (including weapons, illicit drugs and aggressive animals).
  • Give us accurate information about your needs, health, medications and supports.
  • Tell us promptly if your circumstances, NDIS plan or contact details change.

7. Our responsibilities

  • Deliver supports safely, respectfully and in line with your goals.
  • Use suitably screened, trained and insured workers.
  • Protect your privacy and personal information.
  • Listen to feedback and respond to complaints fairly and promptly.

8. Workplace health and safety

If a worker reasonably believes their safety is at risk, they may pause or end a shift and leave the premises. We will discuss any safety concerns with you and work together to resolve them.

9. Termination

Either party may end the service relationship by giving at least 14 days' written notice. We may end services immediately where there is a serious risk to health or safety, fraud, or a fundamental breakdown in the working relationship.

10. Liability

To the extent permitted by law, our liability for any loss or damage arising out of the supply of services is limited to re-supplying the services or paying for them to be re-supplied. Nothing in these Terms excludes any rights you have under the Australian Consumer Law.

11. Changes to these Terms

We may update these Terms from time to time. The current version will always be available on this website. Material changes will be communicated to active clients in writing.
